Institutional change and managerial practices do not spare the places of life that are Institutions
of Accommodation for Elderly Dependent (EHPAD). This contribution aims to clarify
what is «to be managed» and to adapt in these institutions. It questions the presence and
effects of the insane processes on the caregivers, their daily work and the failures of this
work. There are many paradoxes and abysses in a place where professional investments are
confronted with psychological disorganization, degradation and death. The counter-investments
are massive: there is no time-out for the caregivers as for the institution. Everyone has
the feeling of being exhausted by the task, trying to gain time while the passing time is obviously
a time that leads the residents to death. The act of doing and acting therefore replace
thought. The realization of the tasks, the organization and the schedules take center stage.
The symbolic dimensions of frameworks and devices, the purpose of what is implemented,
the meaning of work, are lost. To the disintegration of the thinking apparatus of the demented
elder responds that of the apparatus of thinking of the professionals and the institution.
The resumption of professionalism requires the support of the group, the devices and the
frameworks that block the arbitrariness and the return of infantile omnipotence. The authors
emphasize the importance of structuring teamwork, framework meetings, building common
representations and reading institutional movements. This is where the management and the
clinic have to find a way to bond. This implies new ways of thinking and working. |
